The FY 2027 ICD-10-CM code set takes effect October 1, 2026 and applies through September 30, 2027. CMS's update adds 190 new diagnosis codes, deletes 30 and revises the descriptions of 4. CMS has posted the code files, addenda and conversion tables on its ICD-10 page.
For professional and outpatient claims, the date of service decides which code set applies. A visit on September 30 uses the FY 2026 codes. A visit on October 1 uses FY 2027 codes. Deleted codes will reject after the effective date, and any EHR pick list, template or superbill that still offers a deleted code will send bad claims until it is fixed.
CMS also posted the ICD-10-PCS update for discharges from October 1, 2026 through March 31, 2027. That matters mostly to facilities, but hospital outpatient and ASC teams should check their procedure code tables too.
Action Required for Billing Teams
- Compare the addenda to your top 50 diagnoses. Pull the new, deleted and revised code lists from CMS and check them against your highest-volume diagnosis codes by claim count.
- Update pick lists, templates and superbills before October 1. Confirm your EHR vendor's release date in writing. If it lags, plan a manual workaround.
- Load the code set into your claim scrubber and test. On October 1, run 20 claims with new codes through the scrubber before releasing the batch.
- Brief providers and coders on new specificity. Send a one-page summary by specialty of the codes that changed, and hold any claim dated October 1 or later that still carries a deleted code.
