CY 2027 RPM/RTM Final Rule Changes
Established-Patient Limit and a Staffing Delay to 2029
Summary of the Rule or Edit Change
The CY 2027 Medicare Physician Fee Schedule final rule (published November 4, 2026) locks in three changes to remote physiologic monitoring (RPM) and remote therapeutic monitoring (RTM) programs. First, RTM is now limited to established patients only, closing off the new-patient-enrollment model some digital health vendors had built their intake funnels around. Second, CMS requires a separately reportable initiating visit before RPM or RTM begins; you can't just drop a device on a patient and start billing monitoring codes without a billable visit anchoring the start of care. Third, CMS updated the valuation of the RPM/RTM code family, so expect the payment amounts themselves to shift, not just the eligibility rules around them.
The part that got real relief in the final rule: the proposed requirement that RPM and RTM work be performed only by practice-employed clinical staff, not outside contractors, does not take effect January 1, 2027 as proposed. CMS delayed it to January 1, 2029. If your monitoring program runs through a third-party staffing vendor or contracted care-management company, you have roughly two more years to either bring that function in-house or build a compliant staffing relationship, instead of scrambling before January.
Action Required for Billing Teams
- Audit your RTM enrollment funnel. If any patients are being enrolled in RTM before an established-patient relationship exists, that enrollment path has to change before January 1, 2027.
- Confirm every RPM/RTM start has a billable initiating visit on the chart. No visit, no monitoring claim.
- Re-check your current RPM/RTM payment assumptions against the updated 2027 valuation once CMS publishes the final fee schedule file; don't carry 2026 payment amounts into your 2027 model unchanged.
- If you use contracted (non-employee) staff for monitoring work, you have until January 1, 2029, not 2027, to resolve it: plan the transition, but don't treat it as a Q1 2027 emergency.
