CPT 2027 Maternity Care Code Overhaul
AMA Replaces the Global OB Package With Phase-Based Reporting: 17 Codes Deleted
Summary of the Rule or Edit Change
The AMA's CPT Editorial Panel approved the biggest change to maternity care coding in years for the 2027 code set, early-released so practices, payers and software vendors have a full year to get ready before the January 1, 2027 effective date. Roughly 35 codes are affected across the maternity care family, with 17 current codes deleted outright.
The old model bundled routine antepartum visits, delivery and postpartum care into a small handful of global codes (the familiar vaginal and cesarean delivery packages). The new model reports maternity care in phases. Antepartum visits are reported as E/M services rather than folded into a global fee. Labor management gets its own new codes, split by straightforward versus complex labor. Delivery is still split by vaginal versus cesarean, but it's billed as its own distinct phase rather than as part of a bundle. Postpartum care is reported separately as E/M.
The stated goal is to make maternity billing reflect the actual complexity and volume of care a patient needed, rather than forcing every pregnancy, from a routine four-visit low-risk course to a high-risk pregnancy with a dozen extra visits, into the same flat global fee. For patients who need more antepartum visits or more complex labor management, the new structure lets practices bill for that additional work instead of absorbing it inside a bundle.
Action Required for Billing Teams
This isn't a quarterly code tweak you can absorb the week it takes effect. A typical OB/GYN practice runs maternity care through scheduling templates, EHR order sets, global-period tracking and payer contracts that all assume the old bundled codes exist. Rebuilding that around phase-based billing, while the global codes you've used for years get deleted, is a charge-master project, not a one-line update.
Run the math on what's at stake before you treat this as a 2027 problem. If your practice delivers 40 babies a month and even a 5% billing-setup error rate on the new codes delays or denies payment on two deliveries a month at a typical global delivery fee in the low thousands of dollars, that's real monthly revenue sitting in rework while the new codes bed in. Practices that wait until December 2026 to look at this will be coding live births against a system nobody has tested.

- Ask each major payer for its transition policy. Don't assume Medicaid, Medicare Advantage and your top three commercial payers will all adopt the new codes on January 1; many payers lag the AMA's effective date.
- Rebuild antepartum, labor and postpartum documentation templates around phase-based reporting instead of a single global note, since each phase now needs its own medical necessity and time/complexity documentation.
- Train scheduling and coding staff together. The phase split changes how a visit gets coded in real time, not just how the final claim looks.
- Test a sample patient's full course (first antepartum visit through postpartum check) in your practice management system before go-live, not after.
